REGISTRATION OPENS JULY 1st!
LAVA BEAR WATER POLO REGISTRATION CHECKLIST
To ensure your player is completely registered for the 2024 water polo season, please make sure the following items are completed:
$300 Registration paid to Lava Bear Water Polo via Stack Team App or check payable to Lava Bear Water Polo
Copy of physical turned in to Board President/VP or emailed to lavabearwaterpolo@gmail.com
Lava Bear Water Polo Policy and Media Consent signature - read thoroughly, new policies in place
Concussion Acknowledgement signature
Purchase team suit via "Store" in Stack Team App (players can try on sample sizes July 8th from 5-6pm at Juniper Swim and Fitness)
Dual Sports Agreement completed and signed IF participating in more than one sport
**Please have players and atleast one guardian download Stack Team App to access, sign and pay for registration, turn into Board President/VP, or email paperwork to lavabearwaterpolo@gmail.com**
